Lake Region Personalized Health Services

By scheduling, purchasing, or receiving services from Lake Region Personalized Health Services (“Lake Region,” “we,” “us,” or “our”), you agree to the following Terms and Conditions. Please read them carefully.

1. Services Provided

Lake Region Personalized Health Services provides nurse-led concierge wellness services, which may include but are not limited to:

  • IV hydration and vitamin therapy

  • Vitamin injections and wellness injections

  • Weight-management support and GLP-1 programs

  • Peptide therapy (via partnered medical providers)

  • Wellness consultations and education

  • Mobile and event-based services

All services are provided in accordance with applicable federal and Pennsylvania state laws and under medical oversight when required.

2. Medical Oversight & Telehealth Requirement

Certain services require:

  • A Good Faith Exam (GFE)

  • A telehealth consultation with a licensed medical provider

First-time clients may not receive treatment until medical clearance is completed and approved. Telehealth fees are separate and non-refundable.

3. Not a Substitute for Primary Care

Lake Region services:

  • Are not intended to diagnose, treat, cure, or prevent disease

  • Do not replace primary care, emergency care, or specialist care

If you experience a medical emergency, call 911 immediately.

4. Client Eligibility

Clients must:

  • Be 18 years or older (or have parent/legal guardian consent if applicable)

  • Complete all required intake forms truthfully and accurately

  • Disclose all medical conditions, medications, allergies, and pregnancy/breastfeeding status

Failure to disclose accurate information may result in refusal of service.

5. Medical Clearance & Right to Refuse Service

Lake Region reserves the right to:

  • Decline or discontinue services based on medical appropriateness

  • Refuse treatment if safety concerns arise

  • Stop treatment at any time if adverse symptoms occur

Your safety is our priority.

6. Mobile Services, Travel & Events

Mobile services may include:

  • Travel fees based on distance

  • Minimum service requirements for group or event bookings

  • Set-up and breakdown time

Clients are responsible for providing a safe, clean, and appropriate environment for services.

7. Appointment Scheduling & Cancellations

  • Appointments must be canceled or rescheduled at least 24 hours in advance

  • Late cancellations or no-shows may result in forfeited deposits or service fees

  • Event bookings may have separate cancellation policies outlined in event contracts

8. Payments & Refund Policy

  • Payment is due at time of booking or service

  • Accepted payment methods will be communicated at booking

  • All sales are final

  • No refunds for completed services, missed appointments, or telehealth visits

Unused services may be credited at Lake Region’s discretion but are not guaranteed.

9. Memberships, Packages & Promotions

  • Memberships and packages are non-transferable

  • Promotional pricing may be limited-time and subject to change

  • Benefits do not roll over unless explicitly stated

Lake Region reserves the right to modify membership terms with notice.

10. Results Disclaimer

Individual results vary. Lake Region does not guarantee outcomes, including:

  • Weight loss

  • Symptom relief

  • Energy levels

  • Wellness improvements

Results depend on individual health status, compliance, and lifestyle factors.

11. Assumption of Risk

By receiving services, you acknowledge:

  • IV therapy, injections, and wellness treatments carry inherent risks

  • Potential side effects include bruising, soreness, dizziness, allergic reactions, or other complications

You voluntarily assume all risks associated with treatment.

12. Limitation of Liability

To the fullest extent permitted by law:

  • Lake Region is not liable for indirect, incidental, or consequential damages

  • Liability is limited to the amount paid for the specific service rendered

13. Privacy & HIPAA Compliance

Lake Region complies with HIPAA regulations.
Your health information will only be shared with authorized providers or as required by law.

14. Photography & Marketing (Optional)

With your consent, Lake Region may use photos or testimonials for marketing.
Consent may be withdrawn at any time in writing.

15. Changes to Terms

Lake Region reserves the right to update these Terms and Conditions at any time.
Continued use of services constitutes acceptance of updated terms.

16. Governing Law

These Terms are governed by the laws of the Commonwealth of Pennsylvania.
